Acting Workshop Signup

I, as stated above, am the parent/legal guardian of child stated above, or give consent on my own behalf, hereby grant permission for my child / or I consent to participate in the acting workshop at the Denair Gaslight Theater. I acknowledge and understand the inherent risks associated with such activities. I agree to hold the Denair Gaslight Theater, its directors, officers, employees, agents, and volunteers harmless from any and all liabilities, losses, damages, costs, or expenses arising from any injury to myself or my child or loss of property during my/his/her participation in this workshop.
Furthermore, I grant permission to the Denair Gaslight Theater to take and use any photographs, films, or likenesses of myself and/or my child taken during the event for promotional, advertising, or any other purpose in any media now known or later developed. I waive any rights, claims, or interests I may have to control the use of my or my child's identity or likeness in the photographs and agree that any uses described herein may be made without compensation or additional consideration to me.
